Abstract

Objective: To improve toxicity assessment and management in oncology patients undergoing chemotherapy through the standardized toxicity scale application and digital monitoring. Methods: A descriptive study was conducted in a public hospital in the state of São Paulo, involving the systematic application of the adapted Common Terminology Criteria for Adverse Events (CTCAE) scale during nursing consultations and follow-up through digital tools. Results: A predominance of gastrointestinal toxicities, particularly diarrhea, was observed, along with an increase in nutritional consultations from 572 in 2023 to 752 in 2024. Conclusion: The strategy proved effective for the early detection of adverse events, improving patient safety and strengthening interdisciplinary care.
