The Primary Health Care Unit conducted a territorial assessment that identified socio-environmental vulnerabilities and potentials within its coverage area, as part of a project integrating health and the environment. Key risks included inadequate waste management, such as informal dumping sites, while social strengths included the presence of Non-Governmental Organizations. Given this scenario, the project aimed to promote socio-environmental improvements, expand healthcare beyond facility walls, and raise community awareness about proper waste disposal. Through intersectoral collaboration between public institutions, community groups, and local residents, actions were defined, including the installation of a handrail to improve accessibility on a staircase and the revitalization of a block affected by irregular waste disposal. The interventions were carried out alongside educational activities with the population. A total of 5,000 pamphlets were produced, and 245 residents received home-based guidance from the community health agents and the environmental promotion agent. The wall revitalization involved 30 graffiti artists. The Green and Healthy Environments Program organized an exhibition table, and the Primary Health Care Unit conducted 120 rapid tests, identifying one positive case of syphilis. The project plans to expand to other areas of the territory.
